This site lets you enter your own personal registration information at your convenience to begin the Intake and Registration process using My ProConnect. My Proconnect is a unique medical health record system that will allow you to enter and retrieve your own health records. This system is uniquely encrypted to protect your privacy. The only way to access your information is to use your personal LOGIN id and password. So please remember your id and password and do not share it with anyone.

How do I start?

The first section contains questions regarding your Personal Contact information and your Insurance information. To begin just click "Start Registration Process" below. Once you have completed the second page, or screen, your record will be saved. Note that the * tells you which boxes you must enter – this is required information. Here is a list of the type of information you will need to gather:

1. Contact information / Employer information
2. Emergency contact information
3. Insurance information - have your card ready

If you are prepared to continue or were instructed to complete the second section regarding youe Medical History and Health Assessment, you can click on the"Continue Registration" link below. Here is a list of the type of information you will need to gather:

1. General health information
2. Past surgical history
3. Medications and allergies

If you do not complete the entire 10 screens, you can return to this system and complete the Intake & Registration at your convenience. Note that the * tells you which boxes you must enter – this is required information.

How do I return?

When you want to return, just enter your username and password on the login page and you will be taken to your own Patient Dashboard where you will click the My Records tab and select Intake & Registration from the drop down box to complete your forms. It's that simple.

Why would I need to return?

You may need to come back to these screens for a number of reasons. It is an option we have provided as a convenience for you in the event you were interrupted or just needed to gather more information. We need some very specific and detailed information about you so take your time to complete the registration before you are seen in our clinic.
